Portal
Once you have scheduled your first appointment, you will receive an invitation to our patient portal to the email address that you provided to us. The invitation will be sent from Optimantra for Integrative Health Practices.
Concierge Fee
Payment of the Concierge Fee must be made in advance of or on arrival at the appointment. Fees are as follows:
- annual fee of $600 ($550.00 if you are a family member of a patient who has paid the full annual fee)
- semi-annual fee of $350.00 ($300.00 if you are a family member of a patient who has paid the semi-annual fee)
The first $350 of any concierge fee is non-refundable.
Your Appointment
Please arrive 10 minutes before your appointment. Please bring your insurance card and your driver’s license/ID. We will send you an email reminder, 72 hours before your appointment time. This email will ask you to confirm your appointment. Please press confirm or you might risk losing your appointment. If you need to change or cancel your appointment, please give us 24 hours’ notice to avoid the cancellation fee of $100.00.
