Call us at 410-648-2555 or email us at firstname.lastname@example.org to schedule an appointment. Prior to your appointment, please sign up for our portal and use it to provide information for your first visit regarding your medications and supplements.
Once you have scheduled your first appointment, you will receive an invitation to our patient portal to the email address that you provided to us. The invitation will be sent from Optimantra for Integrative Health Practices.
In the portal you may also communicate directly with Dr. Rollow, Dr. Cooper, or our PAs, Tammy Jett-Palmer, and Maria Shesiuk.
Please be prepared to pay your concierge fee at the time of your appointment if you have not already done so.
- annual fee of $600 ($550.00 if you are a family member of a patient who has paid the full annual fee)
- semi-annual fee of $350.00 ($300.00 if you are a family member of a patient who has paid the semi-annual fee)
Please arrive 15 minutes before your appointment. Please bring your insurance card and your driver’s license/ID. We will send you an email reminder, 72 hours before your appointment time. This email will ask you to confirm your appointment. Please press confirm or you might risk losing your appointment. If you need to change or cancel your appointment, please give us 24 hours’ notice to avoid the cancellation fee of $100.00.
